If you would like to make a donation in memory of a loved one, please use the online form on this page or download a donation form.

Alternatively you can post donations to Diabetes SA, GPO Box 1930, Adelaide, SA 5001.

If you are making a donation on behalf of a friend who has lost a loved one, we will write to the family to inform them of your kind and generous gift (we won’t mention the amount donated).

Diabetes SA provides donation envelopes for attendees at memorial services to make an in-memoriam donation.

Envelopes can be ordered by emailing fundraising@diabetessa.com.au or by phoning 1300 198 204. Many Funeral Directors already have Diabetes SA envelopes but we will gladly provide more on request.
