How to Register

Registering with the NDSS is a positive step in the management of your diabetes. It is free and only has to be done once.

Process for Registration

If you have been diagnosed with diabetes and would like to access the NDSS you must:

  • complete an NDSS Registration Form,
  • or if you have gestational diabetes complete a icon pdf2 Gestational Diabetes Registration Form,
  • have it signed by your Doctor or a Credentialled Diabetes Educator (CDE),
  • mail it to Diabetes SA, GPO Box 9824, Adelaide SA 5001, or visit Diabetes SA 159 Sir Donald Bradman Drive, Hilton 5033.

    Cost of Registration

    There is no charge to register with the NDSS, and the process only needs to be done once.

    Registration Form

    To register for the NDSS, you need to complete and submit an NDSS Registration Form or a icon pdf2 Gestational Diabetes Registration Form. Section 3 of the Registration Form needs to be completed by your Doctor or a Credentialed Diabetes Educator (CDE).

    Information & Starter Pack

    Shortly after you first register with the NDSS, you will receive a Registration Card and a Starter Pack.

    The pack aims to give new Registrants information about the NDSS, including the services and products available to Registrants, and provides general information about diabetes.
