Change of Contact Details

It is important that Diabetes SA has an up-to-date record of contact details for Registrants. If you change your address, phone number, or medical situation, you should complete a Change of Details Form (link to pdf) and send it to Diabetes SA, GPO Box 9824 5001.

Misplaced, Lost or Damaged NDSS Card?

If you have misplaced your NDSS card or it is damaged, contact Diabetes SA to organise a new one. You can phone Diabetes SA during business hours on 1300 136 588 to organise a new card.
